Pre-Hospital Emergency Care (PHEC) Course

This three-day course focuses on the emergency care of adults in the pre-hospital environment. Taught at Skills for Health Level 6 (Enhanced Practice) it is aimed at registered or trainee healthcare professionals and suitably experienced responders developing enhanced competence in pre-hospital emergency and trauma care. The PHEC course is highly practical involving lots of scenario work, supported by peer learning and continuous feedback delivered through a mentor system with our experienced multi-disciplinary teaching faculty. It involves working outside, come rain or shine.

This course is one of only two courses approved by the Sport Grounds Safety Authority (Green Guide) to meet the requirements for those providing crowd medical care. The content and delivery is also endorsed by The Royal College of Surgeons of Edinburgh.

The PHEC course aims to:

  • Equip participants with the knowledge and skills to assess and manage acutely ill or injured adult patients safely in the pre-hospital setting.
  • Develop structured patient-assessment and decision-making skills using recognised frameworks.
  • Enable participants to undertake safe airway, breathing, circulation, disability, and exposure management at an enhanced-practice level.
  • Introduce learners to scene management, personal safety, and multi-agency interoperability (JESIP principles).
  • Promote understanding of pharmacological strategies for emergency care, including analgesia, anaphylaxis, and cardiac-arrest management.
  • Develop appreciation of the governance, planning, and leadership responsibilities involved in providing medical cover at public and sporting events.

On completion, participants will be able to:

  1. Conduct a structured assessment and manage life-threats.
  2. Demonstrate safe airway management up to supraglottic level, basic ventilation, and recognition of airway compromise.
  3. Manage respiratory and circulatory emergencies, including AED use, haemorrhage control, and IV/IO access under supervision.
  4. Apply immobilisation and fracture-management techniques, including safe reduction where indicated.
  5. Recognise and manage medical and traumatic emergencies in adult patients.
  6. Apply JESIP principles and scene-safety procedures in multi-agency environments.
  7. Demonstrate knowledge of pharmacology relevant to pre-hospital care, including safe administration, contraindications, and documentation.
  8. Communicate effectively, maintain accurate handover and documentation, and uphold governance and ethical standards.
  9. Understand the relevance of the Event Safety Guide (Purple Guide) and Guide to Safety at Sports Grounds (Green Guide) as they relate to the role of the Event Doctor/Event Practitioner, including planning, risk assessment, and spectator safety.

This is an ideal course for health care professionals looking to broaden their experience of the pre-hospital emergency care of adults. You may be working in. or preparing to work in pre-hospital emergency care, such as those planning to progress to PHEM training programmes. You may also be preparing, or working within, the role of Event Doctor, Paramedic or Nurse or as a member of the wider professional medical team. The course is also useful for GPs or other health care professionals wanting to add to their pre-hospital knowledge and experience, especially if moving to a rural setting.

Learners should be:

  • Registered healthcare professionals (e.g., nurse, paramedic, physician associate, doctor) or trained responders with relevant pre-hospital experience (e.g. British Army CMTs, Royal Navy MAs or RAF Medics, with senior trade training, who are supported by their command to attend). Other trained responders should be able to demonstrate prior learning equivalent to Level 5 or above on the Skills for Health framework.
  • Familiar with the pre-hospital ABCDE algorithm.
  • Foundational proficiencies in basic life support (BLS), for example a current BLS certificate, use of an AED and physical assessment skills. Whilst not a requirement, ILS and ALS proficiencies are desirable.
  • Able to participate in practical scenarios which will include manual handling and working at floor level.

International students

The course will be taught in English and will normally refer to guidelines and practice norms within the UK healthcare system. The same entry requirements apply for international students.
